Serious illnesses: Heart disease & surgery (pacemaker, angioplasty), Cancer treatments, Kidney/liver issues (dialysis, transplant), Orthopaedic surgeries, Thalassemia, Bone marrow transplant, AIDS, and mental health conditions requiring surgery
Accident-related injuries: Road, rail, farm, industrial, rail mishaps, bomb blasts, etc.
Essential diagnostics & medications: Ultrasounds, CT/MRI scans, angiography, immunosuppressants, plasma/blood products, TB drugs, and more
How Much Financial Support?
Up to ₹2 lakh: Approved by Committee Chairperson
₹2 lakh – ₹3 lakh: Requires State-level Committee approval
Advanced approval possible for hospital payments up to ₹3 lakh

Collect documents:
Passport photo (doctor-attested)
Income certificate (by Revenue Officer)
ID and address proof
Hospital bills, prescriptions, discharge summary
Bank passbook copy
PM-JAY / AAA card if applicable
Submit to NHM Assam: either via district Joint Director Health Services or Mission Director’s office in Guwahati
Verification: A panel of specialists from Gauhati Medical College reviews applications
Fund transfer: Approved payments made directly to your bank or hospital; advance payments allowed up to ₹3 lakh
